In 2022 the World Health Organization published its first-ever fungal priority pathogens list, a milestone for a field that has long been under-resourced relative to bacterial and viral threats.
Four organisms occupy the critical group: Cryptococcus neoformans, Candida auris, Aspergillus fumigatus, and Candida albicans. Each combines serious clinical impact with challenges in treatment or diagnosis.
The list underscores structural problems. There are only a handful of antifungal drug classes, resistance is rising, and rapid, accessible diagnostics remain limited in many settings.
For clinicians, the message is to keep invasive fungal disease on the differential in immunocompromised and critically ill patients, and to pursue diagnosis aggressively, because delays cost lives.
